The Epstein Barr virus (EBV) is a Gammaherpesvirus with a double-stranded linear DNA genome, which infects more than 90% of the world population and is known as the etiological agent of Infectious Mononucleosis (IM).
For a low percentage of immunocompromised subjects an association of the EBV infection with two types of neoplasm has been shown: Burkitt’s Lymphoma and undifferentiated nasopharyngeal carcinoma.
Moreover, recent studies have shown that EBV is involved in pathogenesis of different neoplasms in subjects with acquired or inborn immunodeficiency, e.g. HIV seropositive subjects and organ and bone marrow transplant patients. Such neoplasms are mostly B-cell lymphomas (both Hodgkin and non-Hodgkin), Post-Transplant Lymphoproliferative Disease (PTLD) and T cell lymphomas (e.g. nasal NK/T cell lymphoma).
The detection and quantification of the viral genome provides a prognostic parameter in patients with EBV-related conditions, as the amount of viral DNA in peripheral blood, in cerebrospinal fluid and in biopsies is correlated with the clinical outcome of the disease. Moreover, the study of viremia may be a useful tool for evaluating the effect of different therapeutical approaches.
